“Resources for the Healthcare Community”

Basic Eligibility For a resident:

Must be independent with:

  • Independent with ambulation, transfers, and toileting including the hygiene parts

Can get some assistance with:

  • Dressing, grooming, eating – allowable diets: Regular, No Added Salt (NAS), No Concentrated Sugars (NCS)

Can get more assistance but cannot depend on total assistance with:

  • Bathing

Can have but must be self-managed:

  • Oxygen no more than 4L – tubing and controls must be self-managed
  • Catheters – must be self-managed

Cannot have:

  • Feeding or breathing tubes
  • Modified consistency diets
  • Wandering issues
  • No current drug use or abuse, No current alcohol abuse issues – look for time of clean and sober from any past issues (more than current hospital stay), preferably have gone through a program
  • No aggressive behaviors

To start the admissions process (or to get feedback on a patient for Rehab or a family) we need:

  • Current diagnosis
  • Current medication list
  • Notes from the most recent doctor’s visit
  • 3 days of nursing/care notes (if applicable)
  • Notes from OT and/or PT (if currently receiving or recently discharged from these services)
  • Face sheet – name, DOB, contact information, insurance, Medicare/Medicaid #’s, income and asset information to determine what programs they may be eligible for or need to be applied for (and from the income and asset info, we provide spreadsheet estimate on how long assets will last and/or how Medicaid will look for them in their specific situation)

This information can be sent back via email, fax or secure download (navihealth, allscripts, etc…)
