Here's what you'll need

Please provide copies of the following:

• Picture ID
• Insurance Cards
• Social Security Card
• Social Security Award Letter
• Check for your non-refundable setup fee, in order to reserve your room of choice. (Amount varies according to room size.)
• Healthcare Proxy*
• Power of Attorney*
• Representative Payee*
• Funeral and Burial Arrangements (irrevocable or revocable)
• Most Recent Bank Account Statement
• Information regarding the Oxygen company you use (if on oxygen)

When you move in, please bring the following:

• Your personal belongings, labeled with your name for housekeeping/laundry purposes
• Current medications along with the original prescriptions
• A check for your first month’s rent (prorated if you move in after the 1st of the month).
